Foundations First™

Basic needs. Vital records. Stability starts here.

Foundations First™ is EMBR88C Change Now's basic needs and documentation support program. This program helps individuals and families address foundational barriers that often prevent access to housing, employment, benefits, education, medical care, and long-term stability.

Stability cannot begin when people are locked out of the paperwork that proves who they are, where they live, and what they qualify for.

Vital Records & Notary Support

As part of Foundations First™, EMBR88C Change Now may coordinate limited donated general notarization support through J&A Notary Vybz™ for eligible participants who need notarized documents connected to vital records, identification, benefits, housing, caregiving, re-entry, or stabilization services. This partnership helps remove practical barriers that can delay access to essential documents, services, and opportunities.

Notary support is offered based on availability, eligibility, and service capacity.

Access After 18™

Bridging the gap from school-based services to adult support.

Access After 18™ is EMBR88C Change Now's disability-responsive transition support program for neurodivergent youth, young adults with disabilities, caregivers, and families navigating the gap between school-based services and adult systems of care.

School-based services may continue through age 22 depending on eligibility, IEP status, diploma pathway, and transition planning. However, families still need support preparing for adult systems before those services change or end. Access After 18™ helps neurodivergent youth, young adults with disabilities, caregivers, and families prepare for the transition from school-based services into adult systems of care, support, employment, benefits, housing stability, and community-based resources.

EMBR88C Change Now does not replace school district services, IEP teams, legal advocacy, or public benefit agencies. We help families understand options, organize next steps, and connect with appropriate adult-service systems and community resources.
